The first questionnaire to come into general use was the Cornell Medical Inde: Devised by Brodman in the late 19do's, it consists of a form containing 195 questions which is given to the patient immediately before the office visit. Fur ther processing is not required, although some efforts have been made to introduce a computer - processed versiog.
