Physical examination in first case made the diagnosis very clear, and it was, that the patient had empyema, with spontaneous opening. The opening was rather lower than us occurs in spontaneous thoracentesis, which may be hind or in front, but in the majority of cases at the latter point. The heart was pushed over to the right side considerably. The left side was retracted.
