The earliest ind ication -- A1 tered rhythm--Its relation to " bruits" -- Discrimination of abnormal sounds -- Friction -- Its character and rhythm -- Endocardial Murmurs -- Their causes -- Regurgitation -- Discrimination of Anaemic Murmur -- Not produced by the Valves -- Its situation -- Its occurrence in Acute Rheumatism -- Valvular insufficiency without disease of Valve -- Progress of Inflammatory Disease -- Consequent enlargement of Heart. The earliest intimation we have of inflammation attacking the heart or pericadium in cases of acute rheumatism, comes in the form of altered rhythm of the heart's sounds. This generally precedes the development of any abnormal sound whatever. It is difficult to express in words what can only be learned by much experience and constant watching; but, in my own experience, there has been generally a period when this condition might be traced with considerable certainty. The pulse is invariably quickened, as soon as the 48 ALTERATION OF RHYTHM. febrile symptoms are developed, and the heart's action is necessarily quick too; but at first the length of each sound and pause, though shortened in exact proportion to the rapidity of the pulse, is not changed in relation to the other intervals. As soon as inflammation commences in the heart-structure, the rhythm which characterizes the succession of sounds and their relative duration is altered, and the first sound is no longer distinguished by its length from the second.
