Transplantation of isolated islets from cadaverpancreas is a promising possibility for the optimaltreatment of type 1 diabetes. However, such anapproach is severely limited by the shortage ofdonor organs. And also the long-term function ofhuman pancreatic islet grafts may depend on theneogenesis of beta cells from epithelial precursorswithin the grafted tissue. Islet neogenesis can beinduced in adult mouse and human pancreatic ductalcell cultures. Using the in vitro model for humanislet neogenesis, we confirm the capacity ofendocrine differentiation from progenitors presentin the adult human pancreas. Ductal celldifferentiation into endocrine cells in transplantedislets may be an important factor in sustaining thelong-term function of islet transplants. Theimmunosuppressive protocol appears to be animportant determinant of long-term clinical isletgraft function.
