A welder can lay a flawless bead and still build the wrong joint, and it is almost never a welding failure. It is a reading failure.
The drawing said something specific about which side of the joint to weld, how deep to prepare it, how far each segment should run, or which face had to be finished flat, and the instruction was read past rather than read.
