A patient's story is rarely contained in one note, one test, or one conversation. It takes shape across changing conditions, people, records, decisions, and care settings. When those pieces are read separately, it can be difficult to see what was known, what changed, who held responsibility, and what still needed attention.
In Clinical Intelligence in Practice, I introduce the Lexcura Clinical Intelligence Model(TM) as a structured way to follow the patient's course and understand the evidence in its proper order. The model grew from my work across healthcare settings and complex medical record review. This book brings its questions into the daily world of physicians and clinicians, where the purpose remains patient care and medical-legal awareness helps make decisions and responsibilities clear.
The seven pillars follow a fixed sequence:
Record Integrity asks whether the source material is dependable, complete enough for the question, and clear about its limits.
Baseline Profiling establishes the patient's usual function, condition, and goals so change has a meaningful comparator.
Imaging Analysis examines what a study and report contribute, within their proper scope.
Timeline Reconstruction places findings, communications, decisions, and responses in sequence, including what was available at each point.
Standard of Care Review considers decisions in the context of the patient, the information then available, and the reasonable options.
Regulatory & Compliance Overlay identifies the laws, regulations, standards, policies, and workflows that actually applied in the setting and at the time.
Causation Mapping considers whether the evidence supports a relationship between care and outcome, while keeping other explanations and uncertainty visible.
Each pillar prepares the ground for the next. The order matters because an unsupported source, an assumed baseline, or an incomplete timeline can distort everything that follows. If new evidence changes an earlier answer, that work should be corrected and later conclusions reconsidered. The sequence stays fixed even as the evidence develops.
Through substantial chapters and worked cases, I show how each pillar operates and why its place in the sequence matters. The examples follow patient-specific change, imaging from report through communication and response, decisions before outcomes are known, the system conditions around care, communication and handoffs, and the difference between what happened next and what the evidence can establish about causation. Diagrams, review questions, case exercises, and a clinical practice toolkit help readers apply the method to different settings.
This is not a clinical protocol, a substitute for professional judgment, or a promise that a complaint, claim, or adverse outcome can be prevented. No framework can provide that guarantee. The model can help make the basis for a decision more visible, show who owns the next step, clarify what remains open, and preserve the context in which care occurred.
I wrote this book for physicians and clinicians who want to keep the patient's story connected, communicate clearly across a care team, and create records that help the next person continue the work. It is also for staff and leaders who review complex events and want a disciplined way to distinguish supported findings from assumptions. The goal is not to write for an imagined courtroom. It is to care for the patient, follow the evidence, and make the work understandable if it is later examined.
Clinical Intelligence in Practice offers a practical path from the available evidence to a careful, patient-centered account-one step at a time, in the order the questions require.
