My parents emigrated to England from the Irish Republic, and we have all lived in Britain for so long that no one can see the join - or can they? Some years ago, my mother was admitted to hospital for a routine operation. Award nurse saw her obviously foreign name on the admissions list, and concluded she must be Asian. Accordingly, she arranged for my mother to have a bed in the same bay as other Asian women, and ordered her a curry for lunch. Part of me was angry at the stereotypical thinking rhat every- one with a strange name must be Asian, but I had to concede that I myself knew little about other cultures. My own patients came from a variety of cultural traditions, but neither I nor my colleagues knew very much about them. What information we had was culled directly from patients and their families in a haphazard 'do this' or 'don't do that' basis. Because we had no understanding of the traditions, staff could become exasperated over what they interpreted as 'awkwardness'.Our inadequacies were further highlighted when patients did not speak English: there was always the question of who could translate for them, and whether they fully understood their illnesses and their treat- ments. In 1986 the Trustees of the Nuffield Provincial Hospitals Trust decided to develop a major core programme of studies concerned with the organization, development and improvement of services for physically disabled and frail elderly people. This programme developed rapidly due to the enthusiastic response of those in the field; and by 1992 the total value of the programme was in excess of GBP2 million and currently totals over GBP3. 2 million. It comprised a cohesive matrix of grants commissioned throughout the United Kingdom for work in identified areas which, taken in conjunction with existing grants, formed a comprehensive programme of much-needed studies and research in this field. The whole was to be underpinned by a major study planned and financed by the Trust but conducted by the Royal College of Physicians to survey the existing services being provided by some 180 health authorities. This study, together with concern for the handicapped schoolleaver, information about the availability of disability services and the care of mild/moderate head-injured persons, formed the prime initial thrust of the programme.In the case of head injury, the Trust was particularly concerned that there was discontinuity of care for many patients and little evidence as to which treatments were effective. Once patients had stabilized in hospital they were discharged into the community under the care of their general practitioner often without a continuing responsibility from the hospital. However, the aims of this book remain the same as for the first edition -to present the reader with current reports of studies in nursing practice which use a model of nursing as their base. As someone who was trained in the clinical sdentific tradition it took me several years to start to appreciate that food was more than a collection of nutrients, and that most people did not make their choices of what to eat on the biologically rational basis of nutritional composition. This realiza tion helped tobring me to an understanding of why people didn't always eat what (I believed) was good for them, and why the patients I had seen in hospital as often as not had failed to follow the dietary advice I had so confidently given. When I entered the field of health education I quickly discovered the farnaus World Health Organization definition of health as being a state of complete physical, mental and social well-being, and not merely the absence of disease. Health was a triangle -and I had been guilty of virtu ally ignoring two sides of that triangle. As I became involved in practical nutrition education initiatives the deficiencies of an approach based on giving information about nutrition and physical health became more and more apparent. The children whom I saw in schools knew exactly what to say when asked to describe a nutritious diet: they could recite the food guide and list rich sources of vitamins and minerals; but none of this intellectual knowledge was reflected in their own actual eating habits. Some of the material, particularly in the final chapter, reflects first thoughts about the impact of the new system on industrial relations. The book arose from my teaching experiences with health service practitioners from several of the NHS trade unions and professional associations. Many of these activists, as well as managers, expressed frustration that there was no single source about some of the issues which concerned them. This book is the result of their anxieties. Throughout, I have assumed that the main thrust of government policy towards the NHS, at least since the early 1980s, has been to sell off important sections of the service to the private sector. There is, I believe, strong evidence for this proposition. My argument, however, is based not only on the evidence of government's will to 'privatize', but also on the behaviour of ministers, senior civil servants and senior NHS managers which adds up to a set of policies and practices which together allow the point that government runs the NHS as if it was going to sell it.
