This item is printed on demand - it takes 3-4 days longer - Neuware -Die vorliegende Monographie ist das Ergebnis ausgedehnter tierexperimenteller Untersu chungen tiber den Einflu~ verschiedener Anaesthetika auf die Kontraktionsdynamik des Herzens sowie einer langjiihrigen klinischen Erfahrung mit der DurchfUhrung von Narkosen bei Patienten mit einer manifesten oder latenten Herzinsufflzienz. Das Buch umfaBt zwei mehr oder weniger gleichgewichtige Komplexe: Der erste Teil wird dem Problem gewidmet, inwieweit sich das Herz-Lungen-Praparat als tierexperimentelles Modell zur Untersuchung der Kontraktionskraft des Herzens tiberhaupt, insbesondere aber bei pharmakologisch induzierten Xnderungen des inotropen Status eignet. Der zweite Teil vermittelt sodann Befunde tiber direkte chronotrope und inotrope Wirkun gen von Narkosemitteln am Herzen, wobei es offenbar darauf ankommen sollte, flir intra venose Anaesthetika und Inhalationsnarkotika einen Vergleichsma~stab zu finden und narkotische Dosisaquivalente festzulegen. Besonders originell ist die Formulierung eines sogenannten kardiotherapeutischen Index, mit dem der Abstand zwischen minimal narkoti scher Konzentration eines Narkotikums und schlidlicher Beeintrachtigung der Ventrikel funktion quantitativ defmierbar ist. Die hier vorgelegte Monographie beeindruckt durch die Sorgfalt im methodischen Ansatz, den Umfang der angestellten Untersuchungen und die kritische Beurteilung der Ergebnisse. Soweit ersichtlich hat es eine mit ahnlicher Konsequenz realisierte vergleichende Beobach tungsreihe zahlreicher Narkotika, insbesondere auch unter Einbeziehung von Inhalations narkotika, bislang nicht gegeben. Dabei ist gerade auch die Synopsis der Befunde von un mittelbarem klinischem Interesse, denn bestimmte Einwirkungen einzelner Narkotika waren zwar bislang im Prinzip langst bekannt, mtissen aber nunmehr im unmittelbaren Vergleich mit anderen Narkotika anders beurteilt oder gewichtet werden. 292 pp. Deutsch.
