Neuware -The idea for this issue arose during a gathering of scholars to com memorate the hundredth anniversary of Moritz Schlick (1882-1936), the philosopher from Germany whose influence gave Austria its most characteristic philosophical voice between the two world wars. He was cut off, tragically, in his prime and while he escaped the exile that awaited most of those who thought like him, he was unable (sadly for philosophy) to continue to steer their thoughts in his own direction and he even lost some of the credit for work already done. Thus it seemed to some of his former pupils and to others more remote from him in the tra dition that a small collection of papers throwing light on his especial con tribution and on the extent to which it is still active or still needed today was a requirement of justice no less than of piety. Tscha Hung, a mem ber of the Vienna Circle and since director of the Institute for Western Philosophy at Peking University, was the chief mover here. Also among the contributors, Ludovico Geymonat (Professor at Milan) was a visitor to the Circle and a friend of Schlick. Henrich Melzer and Joseph Schlichter were Viennese pupils of Schlick's. How should we spend our tax d'ollars, and who should get help The purpose of this volume is to reflect upon our choices. The authors whose papers appear herein identify major difficulties and offer various solutions to them. Four topics are discussed throughout the volume: First, encounters between patients and health professionals in critical situations in general, and where scarcity makes rationing necessary; second, allocation and social policy, including how much to spend on preventive, chronic or critical care medicine, or for medicine in general compared to other important social projects; third, conflicts between or ranking of important goals and values; and fourth, conceptual issues affecting the choices we make. Since these topics are raised by the authors in almost every essay, we did not divide the papers into separate sections within the volume. Warren Reich begins the volume with a parable illustrating a key problem for contemporary medicine and two very different approaches to its solution. His story begins with the 'delivery' of three indigent, critically ill, foreign patients to the emergency room of a large American private hospital. His General Theory of Relativity dealt with the nature of this interaction in the context of gravitational fields, and led to a view of the universe which was soon observationally confirmed. Initially, my plan was to discuss both the fundamental organometallic chemistry and applications in organic synthesis. When the first draft of the manuscript was complete, it was apparent that I had exceeded my allowance of pages by a huge amount. It was then that I decided that the catalysis section warranted separate treatment. I am grateful to Reidel for agreeing to publish this volume on Homogeneous Catalysis with Compounds of Rhodium and Iridium as part of their 'Catalysis by Metal Complexes' series. The material I had for the original Academic Press project covered the litera ture to the end of 1978. I decided to update this to the end of 1982 with a few key references from 1983. For a variety of rather complex reasons we continue to split apart and treat as radical opposites body and spirit, medicine and religion, sacred and secular, private and public, love and justice, men and women. Though this is still our strong tendency, we are beginning to discover both the futility and the harm of such dualistic splitting. Peoples of many ancient cultures might smile at the belatedness of our discovery concerning the commonalities of medicine and religion. A cur sory glance back at ancient Egypt, Samaria, Babylonia, Persia, Greece, and Rome would disclose a common thread - the close union of religion and medicine. Both were centrally concerned with healing, health, and wholeness. The person was understood as a unity of body, mind, and spirit. The priest and the physician frequently were combined in the same individual. One of the important contributions of this significant volume of essays is the sustained attack upon dualism. From a variety of vantage points, virtually all of the authors unmask the varied manifestations of dualism in religion and medicine, urging a more holistic approach. A spate of new papers has been published on well known and less familiar syndromes, and there have been a number of attempts to put some order into the field of inquiry. In a review of the literature on culture-bound syndromes up to 1969 Yap made certain suggestions for organizing thinking about them which for the most part have not received general acceptance (see Carr, this volume, p. 199). Through the seventies new descriptive and conceptual work was scarce, but in the last few years books and papers discussing the field were authored or edited by Tseng and McDermott (1981), AI-Issa (1982), Friedman and Faguet (1982) and Murphy (1982). In 1983 Favazza summarized his understanding of the state of current thinking for the fourth edition of the Comprehensive Textbook of Psychiatry, and a symposium on culture-bound syndromes was organized by Kenny for the Eighth International Congress of Anthropology and Ethnology. The strong est impression to emerge from all this recent work is that there is no substantive consensus, and that the very concept, 'culture-bound syndrome' could well use some serious reconsideration. Knowledge of resistance mechanisms, and of how to exploit them, has made a significant contribution to agricultural productivity. However, the continuous evolution of new variants of pathogen, ana additional control problems posed by new crops and agricultural methods, creates a need for a corresponding increase in our understanding of resistance and ability to utilize it. The study of resistance mechanisms also has attractions from a purely academic point of view. First there is the breadth of the problem, which can be approached at the genetical, molecular, cellular, whole plant or population lev~ls. Often there is the possibility of productive exchange of ideas between different disciplines. Then there is the fact that despite recent advances, many of the mechanisms involved have still to be fully elucidated. Neuware -It goes almost without saying that there has been a marked increase in the incidence of sexually transmitted diseases throughout the world in the past two to three decades. Indeed, despite the progress that has been made in methods of diagnosis and treatment, the sexually transmitted diseases as a whole are the most common communicable diseases and as such constitute an important health problem. The increase in incidence may be accounted for by changes in sexual behaviour, the introduction of contraceptives and the increasing mobility of the population. In addition, during the same time period, the number of infectious agents recognized as being sexually transmitted has increased considerably. These include Chlamydia trachomatis, herpes simplex virus, cytomegalovirus and hepatitis B virus. Indeed, some are as dependent on sexual transmission as the agents which cause the traditional venereal diseases and collectively they cause morbidity which has out-stripped that caused by gonorrhoea and syphilis. It could almost be said that to know the sexually transmitted diseases is to know micro biology. However, the approach taken in this book has not been to consider individual infectious agents and evaluate what they do and do not cause but to consider clinical conditions and what might be responsible for them. On the theoretical side, this symposium saw the first direct evidence of gravothermal oscillation through N-body simulation, which was made possible by GRAPE-4, the dedicated special-purpose computer for N-body simulation. Manning 'Can you prove that continuing education really makes any difference ' Over the years, educators concerned with continuing education (CE) for health professionals have either heard or voiced that question in one form or another more than once. But because of the difficulty in measuring the specific effects of a given course, program, or conference, the question has not been answered satisfactorily. Since CE is costly, since CE is now mandated in some states for re-registration, and since its worth has not been proven in for mal evaluation research, the pressure to evaluate remains strong. He was cut off, tragically, in his prime and while he escaped the exile that awaited most of those who thought like him, he was unable (sadly for philosophy) to continue to steer their thoughts in his own direction and he even lost some of the credit for work already done. Thus it seemed to some of his former pupils and to others more remote from him in the tra dition that a small collection of papers throwing light on his especial con tribution and on the extent to which it is still active or still needed today was a requirement of justice no less than of piety. Tscha Hung, a mem ber of the Vienna Circle and since director of the Institute for Western Philosophy at Peking University, was the chief mover here. Also among the contributors, Ludovico Geymonat (Professor at Milan) was a visitor to the Circle and a friend of Schlick. Henrich Melzer and Joseph Schlichter were Viennese pupils of Schlick's.
