In the past two decades we have witnessed very significant advances in the management of the very ill patient. The great success in this field of medical endeavour is largely due to the establishment of intensive care units, but a great deal of progress can also be attributed to the major developments in technology, which affect patient management and care as well as the many sophisticated techniques of diagnosis and patient monitoring. Imaging and Labelling Techniques in the Critically III covers this new important and difficult field of diagnosis and visual monitoring. By establishing the criteria and algorhythms for the choice of the different methods available for this purpose, defining the diagnostic signs on images and resolving some of the mis conceptions and pitfalls, this book will go a long way to help the reader, particularly those involved in the care of patients in the intensive care units. This book brings together many different methods of investigation and discusses the advantages and limitations of these techniques in different clinical circumstances. Some of the techniques are well established and their usefulness in the intensive care unit is in no doubt. Some of the newer techniques such as PET scanning or NMR imaging have not yet found a defined position of usage in the critically ill patient. There is, however, little doubt that in due course this situation will change.

For a long period after the ~scovery of the photoelectric effect. it was not possible to generate electro magnetic fields that did more than perturb the atom only slightly, and (first-or~er) perturbation theory could perfectly explain what was going on at those low intensities. The development of the pulsed laser bas changed this state of affairs in a rather dramatic way, and fields can be applied that really have a large, or even dominant influence on atomic structure. In the latter case, w~ speak of super-intense fields. Since the interaction between atoms and electromagnetic waves is characterized by many parameters other than the light intensity, such as frequency, iQnization potential, orbit time, etc., it is actually quite difficult to define what is exactly meant by the term 'super-intense'. Obviously the term does not have an absolute meaning, and intensity should always be viewed in relation to other properties of the system. An atom in a radiation field can thus best be described in terms of various ratios of the quantities involved. The nature of the system sometimes drastically changes if the value of one of these parameters exceeds a certain critical value, and the new regime could be called super-intense with respect to that parameter.

For example, on the eve of the March 19, 1926 Chungshan gunboat incident at Canton, an incident in which the Communists had plotted to kidnap General Chiang Kai-shek, then Commandant of the Whampoa Military Academy, someone had asked the General himself in person whether he was going back to Whampoa that day. Three telephone calls were made asking this question. In making a report of the incident after it was over, General Chiang did not identify who the individual was who was so persistent in ascertaining the General's movements on that momentous day, nor did he ever breathe a word of it even to his closest aides. Most of this increase has occurred since the beginning of the present century (Murozami et 01. , 1969), and taken into perspective, a typical individual living in the industrialized world sustains a lead burden 500 times that of his prehistoric ancestors (Patterson, 1983). Lead is unique in being an environmental pollutant where the levels deemed clinically toxic are less than an order of magnitude from those that are normally encountered in the population. The clinical limit for lead exposure, 70 JLg Pb/tOO ml blood, is about three times the 'normal' value (in the range 15-30 JLg Pb/100ml blood). This small difference is remarkable from the toxicological standpoint. The key concept here is control of dis persal. The industrial actions of man bring contact with a wide variety of substances, some of which are poisonous or dangerous. In such cases pro tective measures are taken to keep hazardous exposure to a minimum. In the of lead compounds, stringent regulations are production and distribution enforced. The exception to this is in the dispersal of lead from vehicle exhausts following purchase from the petrol pump. Perhaps man's long standing acquaintance with lead in home and industry is responsible for a complacency which until only recently has been unquestioned. Beginning in the late 1960s, evidence began to accumulate suggesting that lead at relatively low levels of exposure might be causing negative effects on neurobiological function.

The common thread in these algorithms is that the distributed system is viewed as a graph, with vertices representing the computing nodes and edges representing some other feature of the system (for instance, point-to-point communication channels or a conflict relationship). Each algorithm assigns a virtual direction to the edges of the graph, producing a directed version of the original graph. As the algorithm proceeds, the virtual directions of some of the links in the graph change in order to accomplish some algorithm-specific goal. The criterion for changing link directions is based on information that is local to a node (such as the node having no outgoing links) and thus this approach scales well, a feature that is desirable for distributed algorithms. This monograph presents, in a tutorial way, a representative sampling of the work on link-reversal-based distributed algorithms. The algorithms considered solve routing, leader election, mutual exclusion, distributed queueing, scheduling, and resource allocation. The algorithms can be roughly divided into two types, those that assume a more abstract graph model of the networks, and those that take into account more realistic details of the system. In particular, these more realistic details include the communication between nodes, which may be through asynchronous message passing, and possible changes in the graph, for instance, due to movement of the nodes. We have not attempted to provide a comprehensive survey of all the literature on these topics. Instead, we have focused in depth on a smaller number of fundamental papers, whose common thread is that link reversal provides a way for nodes in the system to observe their local neighborhoods, take only local actions, and yet cause global problems to be solved. We conjecture that future interesting uses of link reversal are yetto be discovered.
