Freud s eighth scientific publication and first documented clinical case study, reporting a case of cerebral hemorrhage in a patient with scurvy. Published in Wiener Medizinische Wochenschrift, Austria s leading medical journal, it marks his transition from neuroanatomical research to clinical neurology, following the advice of his mentor Ernst Brücke, who urged him to shift toward clinical medicine for academic recognition. This early study highlights Freud s detailed observational approach before his later work in psychoanalysis. In a letter to his fiancée, Martha Bernays (January 18, 1884), Freud described his close involvement in the case, carefully tracking the patient s symptoms, including symmetrical paralysis and rapid decline. He anticipated that the autopsy findings would confirm his diagnosis, justifying publication. This study was an important step in his early medical career, aligning with his growing focus on correlating clinical symptoms with post-mortem pathology. Prior to this publication, Freud s research centered on neuroanatomy and histology, particularly the structure of nerve fibers in aquatic species. His work demonstrated fibrillary nerve structures and laid the foundation for later studies on neuropathology and cerebral localization. Shortly after, he developed a gold chloride staining technique for nerve fibers, published in Brain: A Journal of Neurology (1884), and continued investigations into the medulla oblongata and spinal pathways. An important contribution to the history of neuroscience and a significant milestone in Freud s scientific output before his transition to psychoanalytic theory.
