Inscribed and signed by Dr. Cutolo on the first page. On the inside of the front cover is a gift inscription from someone else who calls Bellevue their home. The dust jacket has wear and chipping along the edges, especially the ends of the spine. Scuffing and soiling. The jacket is in new mylar.
