Single one-sided sheet 8.5" x 11". "Permission to publish and reprint" form with note written by Mamet in red felt marker. Note reads, "I am sorry I can not reply to your request favorably. My letter was for Mr. Kelly's eyes only and not intended for publication. D. M." Stapled to this letter is a 3.5" x 7" card written by Harriet Voyt, Mamet's assistant, which reads, "Dear Ms. Odland, Enclosed please find David Mamet's response to your request. Best regards, Harriet Voyt." It is dated 10-27-94. The card includes Mamet's chop and Voyt's printed name. The letter has two horizontal folds. ; 4to 11" - 13" tall; Signed by Author.
