Endobronchial brachytherapy is an established modality for palliation of symptoms, namely dyspnoea, haemoptysis and cough that result from disease infiltration of the airways. This study was aimed at evaluating the role of HDR endobronchial brachytherapy with external radiotherapy for symptomatic improvement and compliance for such patients at our institution. Total 50 patients were randomised into two arms according to presenting symptoms and Bronchoscopic feasibility for the catheter insertion. There were 20 patients in arm A and 30 patients in arm B. In short follow up, complete response for haemoptysis was achieved in 98% of patients (n=14), 80% patients showed complete response to dyspnoea (n=46). Symptoms like chest pain and cough showed less improvement after endobronchial brachytherapy. Re-expansion of atelectasis was seen in 100% patients (n=29). La braquiterapia endobronquial es una modalidad establecida para paliar los síntomas, a saber, la disnea, la hemoptisis y la tos que resultan de la infiltración de las vías respiratorias por la enfermedad. El objetivo de este estudio era evaluar el papel de la braquiterapia endobronquial HDR con radioterapia externa para la mejora sintomática y el cumplimiento terapéutico de estos pacientes en nuestro centro. Se aleatorizó a un total de 50 pacientes en dos brazos según los síntomas que presentaban y la viabilidad broncoscópica para la inserción del catéter. Hubo 20 pacientes en el brazo A y 30 pacientes en el brazo B. En un seguimiento breve, se consiguió una respuesta completa a la hemoptisis en el 98% de los pacientes (n=14), el 80% de los pacientes mostraron una respuesta completa a la disnea (n=46). Los síntomas como el dolor torácico y la tos mostraron menos mejoría tras la braquiterapia endobronquial. La reexpansión de la atelectasia se observó en el 100% de los pacientes (n=29). Au total, 50 patients ont été répartis de manière aléatoire en deux groupes en fonction des symptômes présentés et de la faisabilité bronchoscopique de l'insertion du cathéter. Dans le cadre d'un suivi court, une réponse complète à l'hémoptysie a été obtenue chez 98 % des patients (n=14), 80 % des patients ont montré une réponse complète à la dyspnée (n=46).
