Ya no existe justificación para los métodos inespecíficos, de amplio espectro y escasa eficacia. También han quedado atrás los tiempos en que se infiltraban… al azar los puntos sensibles, bien con una solución anestésica local, bien con una suspensión de hidrocortisona. Gracias a un mejor diagnóstico, se están reemplazando esas medidas espectaculares por un tratamiento dirigido a la lesión. Los fisioterapeutas utilizamos la manipulación con un fin determinado y administramos la fricción profunda en lesiones exactamente localizadas. También los médicos rehabilitadores emplean la manipulación y las inyecciones en un punto definido dentro de la estructura afectada. Por primera vez se reúnen ambos aspectos del mismo problema terapéutico (médico y fisioterápico) con el resultado de que la manipulación, la tracción, las inyecciones y la fricción profunda puedan abordarse desde las dos vertientes. La totalidad de los métodos manipulativos de la columna descritos aquí están específicamente referidos al disco intervertebral. No se basan en la idea osteopática de restablecer la amplitud normal del movimiento en una articulación vertebral, ni tampoco de la noción quiropráctica de la vértebra desplazada, por lo que pueden ser aplicados sobre personas sanas.
