Assessment of hepatic dysfunction varies according to clinical circumstance - from the asymptotic outpatient, to the patient with severe medical illness. Hepatic masses are frequently discovered during abdominal imaging, often for unrelated complaints, and require appropriate evaluation. Preoperative clearance for surgery of a patient with underlying liver disease is important to assess perioperative risk and likelihood of postoperative hepatic decompensation. Increasing incidence and expanding treatment options for hepatocellular carcinoma has made management of this tumor a prominent part of current hepatology practice. The increasing prominence of nonalcoholic fatty liver disease is addressed by Drs. Rafiq and Younossi. Although alcoholic liver disease continues to be a major challenge, there is renewed interest in its medical management, as well as appropriate indications for liver transplantation in abstinent patients. Dr MacClain and colleagues address an issue of increasing concern, namely hepatotoxicity due to not only therapeutic agents but also herbal compounds. Newer immunosuppressants and other advances in the management of autoimmune and cholestatic liver diseases are reviewed by Drs. Krok and Munoz. An update on hepatitis vaccination is provided by Drs. Nguyen and Tran; whilst another paper discusses the important role of the gastroenterologist and hepatologists in caring for the cirrhotic patient who may ultimately be bound for liver transplant.
